Let’s review a checklist of the items you will need for your first visit:
1) Completed New Patient Forms
- If you cannot print out the forms, we will gladly supply you with a copy to fill out in the office when you arrive. However, please arrive early enough to do so before your scheduled appointment time.
2) Dental insurance/plan information
- We are contracted with a number of dental insurances. An updated list will be uploaded soon.

3) Referral Slip
If you are being referred to us by another dentist, we ask that the dentist fill out our referral slip on the right, so we may better understand your periodontal needs.
Important: Please notify us of any health predispositions that require pre-medication or special attention/assistance prior to your arrival at our office.
During the exam, our assistants will need to take further diagnostic x-rays and new patient photos to superficially establish your initial case presentation. Dr. Abdoney will consult with you on what treatment he recommends based on what he observes in the exam.
Occasionally, we will perform a procedure on the same day as a new patient exam depending on the case. Please notify us of your intention to receive treatment the same day as your initial exam so that we may allow ample time in our schedule.
